If  you are a veterinary surgeon and wish to refer a behavioural case I will require:

A copy of the medical records

A completed referral form

 

Without these details, we will be UNABLE to process the referral.​

 

On receiving this information we will write to the client within 14 days and inform them of what the process is, how we can help, and request more in-depth information on the nature of the problems they are experiencing.
